It may also be a way to reap quite a nice tax benefit. If that office would usually lease for (making numbers up here) 10 thousand per month, that's a 120k per year loss/charitible donation he can take against his new salary.
That of course, assumes that there is a Physician wanting to lease the office. This may also be a way to receive a benefit while waiting on a paying tenant. And to make myself clear, I applaud the 1 dollar lease.

True, but still not nearly as profitable (net) as leasing it to another M.D. Let's not let his financial sophistication diminish his altruism.
